A subsidy health and safety inspection (SHSI) was initiated on 05/19/2025 and completed on 05/19/2025. The inspection stated at 11:52 am and concluded on 1:55 pm. There was 1 child present and 1 caregiver. The inspector reviewed 1 child's record and 1 record for the caregiver and 1 household member on-site.
