During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 8/12/25 and completed on 8/16/25, as a part of, the conditional licensure period. The on-site inspection began at 9;17 AM and ended at 10:25 AM. The inspector reviewed compliance in the areas listed above. There were 1 child present and 1 caregivers; the point total was 4. The inspector reviewed 4 children?s records on-site.
This inspection included; document review, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/22/25. A POC submitted after this date will not appear on the public website.
Violations
7under the direction of a physician prior to a child's attendance or within 30 days
after the first day of attendance.
*Child #1 had no documentation of a completed physical on file. (Overdue by 2 days)
*Children #2, 3 had documentation of a completed physical on file. (Overdue by 2 weeks)
*Child #1's record was missing: Verification of Proof of Birth (overdue by 3 weeks), Authorization for emergency care (overdue by 1 month)
*Children #2 and 3's records were missing: 2 emergency contact's address (overdue by 6 weeks).
*Child #4's record was missing: 1 parent's work address, First day of attendance.
4 of 4 child records reviewed were missing written acknowledgment of the required policies and procedures. (Children #1,2,3,4)
Written parent authorization noting any known adverse reactions shall be
obtained at least annually.
4 of 4 child records reviewed did not contain authorization to apply a topical diaper cream.

There was no documentation available of practiced emergency drills
documentation that the child has been adequately immunized according to the
requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of
Health regulations.
*Child #4 had no immunization record on file.
Children #2 and 3 were overdue for immunization updates by 6 weeks.
