Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/12/25 and completed on 8/16/25, as a part of, the conditional licensure period. The on-site inspection began at 9;17 AM and ended at 10:25 AM. The inspector reviewed compliance in the areas listed above. There were 1 child present and 1 caregivers; the point total was 4. The inspector reviewed 4 children?s records on-site.
This inspection included; document review, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/22/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or
under the direction of a physician prior to a child's attendance or within 30 days
after the first day of attendance.

A child's record shall contain the following information; first date of attendance, information for 2 emergency contacts, parent's address, proof of birth documentation, authorization for emergency care
