The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
1. Child #2, who has been attending for 6 weeks, did not have documentation of an immunization record on file.
2. Child #3, who has been attending for 6 weeks, did not have documentation of an immunization record on file.
3. Child #8, who has been attending for 7 weeks, did not have documentation of an immunization record on file.
4. Child #11, who has been attending for 4 months, did not have documentation of an immunization record on file.
5. Child #12, who has been attending for 7 weeks, did not have documentation of an immunization record on file.

Nonprescription medication shall be in the original container with the direction label attached.

Repeat Violation
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.
Medication for child #15 was on site, but the medication authorization expired 61 days ago. The authorization had not been renewed and the medication was not returned or disposed of as required.

2. The record for child #8 did not contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. The record for child #8 did not contain documentation of viewing proof of the child's identity and age.
3. The record for child #10 did not contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
