During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on 9/17/25 and completed on 9/17/25, as a part of the licensure period. The on-site inspection began at 11:21am and ended at 12:25pm. The inspector reviewed compliance in the areas listed above. There were three children present and one caregiver; the point total was ten. The inspector reviewed three children?s records and five caregiver/household members records on-site. This inspection included document review and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1Children's records are required to contain emergency contact information and the name and
policy number of the child's medical insurance.
Child A and Child B, enrolled for three days, are missing the addresses for the first and second emergency contacts. Child C, enrolled for 29 days, is missing the policy number of their medical insurance.
