During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An announced, on-site initial inspection was initiated and completed on 4/21/25. The on-site inspection began at 9 am and ended at 11:10 am. The inspector reviewed compliance in the areas listed above. There are no children enrolled as the center is not in operation at this time. The inspector reviewed 4 staff files on site. This inspection included document review, tour of the facility, interviews and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and the violation is documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/29/25. . A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to protruding nails, bolts or other components that could entangle clothing or snag skin.

There were protruding screws more than 2 threads along the chain link fence and gate that separate the two playgrounds.
Plan of Correction: There is a plan in place with construction team to come and shave down the bolts.
