During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/22/25, as a part of the conditional licensure period. The on-site inspection began at 9am and ended at 11:50 am. The inspector reviewed compliance in the areas listed above. There were 49 children present and 10 staff. The inspector reviewed 7 children?s records and 6 staff records on-site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/18/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.

Child A has an allergy to almonds, this allergy was not listed on the 7/22/25 allergy list.
Plan of Correction: Parents have given updated paperwork from the doctor.
Standard 8VAC20-780-510-E
The center's procedures for administering medication shall include duration of the parents authorization for medication, provided that it shall expire or be renewed after 10 work days. Long term prescription drug use and over the counter medication may be allowed with written authorization from the child's physician and parent.

Th parent authorization for over the counter medication had not been renewed every 10 work days nor was their a physicians long term authorization for a medication for child A. The parent authorization had expired 7 week prior..
Plan of Correction: Moving forward make sure our paperwork is always up to date with parent and doctor.
Standard 8VAC20-780-510-I
The center may administer prescription medication that would normally be administered by a parent /guardian to a child provided the center has obtained written authorization from a parent/guardian.

The center's medication authorization forms used to obtain medication authorization does not specifically identify the center by name as being given the authority to administer the medications for child A and B. This information has been missing for over two months.
Plan of Correction: We ordered a stamp for us to use so the parent and doctor can give us permission to administer medications.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include a place to document ways to prevent reoccurrence.

May and some of the June 2025 injury forms used by the center did not contain a place to document ways to prevent reoccurrence .
Plan of Correction: On our incident reports there is a spot to write how we plan to prevent moving forward.
