During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 10/15/25. The on-site inspection began at 2 pm and ended at 4:15 pm. The inspector reviewed compliance in the areas listed above. There were 61 children present and 8 staff. The inspector reviewed 4 children?s records and 4 staff records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/6/25. A POC submitted after this date will not appear on the public website.
Violations
4There was no immunization record on file for child C. These records were overdue by 3 months.
The immunization record for child B did not have a signature by the physician or their designee. This signature was overdue by 5 months.
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.
The posted allergy listed did not list child A's diagnosed nut allergy. This was overdue by 5 months.
The center's procedures for administering medication shall include duration of the parents authorization for medication provided that it shall expire or be renewed after 10 work days. Long term prescription drug use and over the counter medication may be allowed with written authorization from the child's physician and parent.
The parent for child A did not renew their authorization for an over the counter medication after 10 work days nor was their written authorization from the child's physician.
