Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on August 14, 2025, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to children's records. The on-site inspection began at 11:55 a.m. and ended at 12:45 p.m. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers; the point total was 29. The inspector reviewed 5 children?s records on-site. This inspection included document review and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
