During the inspection, the inspector reviewed the areas listed above. Sme standards could not be inspected until the program is licensed and operating. Those standards will be reviewed at the first monitoring inspection during the six-month conditional licensure period. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An announced, on-site, initial monitoring inspection was initiated and completed on 6/10/20205. The on-site inspection began at 10:00 am and ended at 11:20 am. The inspector reviewed compliance in the areas listed above.

There were no children or staff, aside from the administrator, present as the program is not yet in operation. Staff and children's records will be reviewed at the first monitoring inspection after the conditional license is issued..

This inspection included document review, tour of the facility, interviews, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-780-330-B
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall.
The fall zone in front of the slide chute measured 8.1 feet when it needs to be at least 8.5 feet.
The fall distance between the slide and the swings measured 7 feet 5 inches when it needs to be 6 feet each for a total of 12 feet of a fall zone.
Plan of Correction: We will fix all the measurements and fall zones. The students will not play on the play area until it is fixed. The students will play on the hardtop and grassy area with outside toys.
