During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site focused inspection was initiated on 8/6/2025 and completed on 8/8/2025 as a part of the conditional licensure period,. The on-site inspection began at 11:45 am and ended at 2:22 pm. The inspector reviewed compliance in the areas listed above. There were 20 children present and 2 staff. The inspector reviewed 5 children?s records and 5 staff records on-site electronically on 8/6/2025. Additional documentation was reviewed electronically on 8/8/2025. This inspection included document review, tour of the facility, interviews, observations, measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. An aerosol can of air freshener with warning label, ?Keep out of reach of children,? was unlocked in the under sink cabinet in the back bathroom designated for staff. This bathroom is unlocked and accessible to children.
Plan of Correction: These items were removed to a locked location during the inspection.

Staff shall request at least annually parent confirmation that the
required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually.
The information in the record of child #1 had not been updated in over 5 years.
The information in the record of child #2 had not been updated in over 10 years.
The information in the record of child #3 had not been updated in over 4 years.
The information in the record of child #4 had not been updated in over 8 years.
The information in the record of child #5 had not been updated in over 6 years.
Plan of Correction: Parents whose children were previously enrolled and transferred to this site will be asked to review their child's paperwork and make any required changes or complete new enrollment paperwork within 2 weeks.
