Inspector Notes

An initial subsidy health and safety inspection (SHSI) completed on 04/02/2025. The inspection started at 1:00 pm and concluded at 2:10 pm. There were no children present and 1 caregivers; the point total was 0. The inspector reviewed 1 children?s records and 2 caregiver/household members records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/10/2025.

Standard 8VAC20-790-260-A
Areas and equipment of the home, inside and outside, shall be maintained in a clean, safe, and operable condition.

The outdoor area had a wire fence, close to the play structure that had sharp edges.
Plan of Correction: We will remove the fencing in the outdoor area.
Standard 8VAC20-790-390-E
The diapering surface shall be nonabsorbent and washable.

The changing pad the provider intends to use for diaper changing is not nonabsorbent.
Plan of Correction: We will buy a new changing mat that is nonabsorbent.
