During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

A subsidy health and safety inspection (SHSI) completed on 03/25/2026. The inspection started at 10:05 am and concluded at 11:00 am. There were 4 children present and 1 caregivers; the point total was 13. The inspector reviewed 5 children?s records and 2 caregiver/household members records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/01/2026.

Standard 8VAC20-790-170-A
The vendor shall ensure compliance with the standards, the terms of the vendor agreement, and all relevant local laws and regulations.

The vendor's permit limits care to the basement of the home, however at the time of the inspection, all 4 children in attendance, and the vendor, were on the upper level of the home.
Plan of Correction: All children activities have been moved back to the approved basement area in accordance with the vendor agreement. Children remain only in the approved area during care hours and strictly follow all licensing requirements moving forward.
Standard 8VAC20-790-260-C-2
Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

There were two outlets without protective covers in both play areas, one was on a wall in a separate room, the second was on a wall with a crib pushed up against it, leaving the outlet accessible to the child in the crib.
Plan of Correction: Protective outlet covers have been installed on all electrical outlets including those in play areas and near cribs. Daily safety checks will be conducted to ensure all outlets remain covered.

Each bathroom area provided for children shall be equipped with soap, toilet paper, and disposable towels within reach of children.

The bathroom in basement, to be used by children in care, did not have soap or disposable towels for children to use.
Plan of Correction: Soap, toilet paper and disposable towels have been placed in the basement bathroom for children's use.
Standard 8VAC20-790-350-G
Cots, rest mats, or beds shall be provided for children 12 months of age or older.

During the inspection, a child (14 months old) was witness sleeping in their car seat with a jacket on, and the straps buckled only on the legs.
Plan of Correction: Effective immediately, all children 12 months and older will be placed on appropriate cots or mats for rest and sleeping time. The use of a car seat for sleeping has been discontinued. Staff will ensure children are dressed appropriately for sleep, no jackets and are always supervised.
