During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/12/2025, as a part of the conditional licensure period. The on-site inspection began at 11:50am and ended at 12:25pm. The inspector reviewed compliance in the areas listed above. There was 1 child present and 1 caregiver; the point total was 2. The inspector reviewed 1 child record on site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-60-B
An up to date record for each child enrolled shall be maintained by the provider and contain all required information. The record for Child #1 did not contain the name, address and telephone number of a second designated person to contact in case of an emergency if the parent cannot be reached.
Plan of Correction: Provider will request the information from the parent.
Standard 8VAC20-800-70-A
Before the child's first day of attendance, parents shall be provided required information in writing.' The parents of Child #1 were not provided in writing the following required information: provisions of the family day home?s emergency preparedness and response plan. Child #1 had been in care for one week.
Plan of Correction: Provider will provide the parent with the form and request parent's signature of receipt.
