Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There were no physical exam records available for Child #1 who was present during the inspection and had been in care for more than 6 weeks. There were no physical exam records available for Child #2 who was present during the inspection and had been in care for more than 3 months.
Plan of Correction: Provider will request a copy of a current physical exam for Child #1 and 2 from their parents.
Standard 8VAC20-800-460-D
The provider shall maintain documentation of the current rabies vaccination. Rabies vaccinations for one pet were overdue by 5 months.
Plan of Correction: Provider will request documentation from the veterinarian.

Repeat violation

An up to date record for each child enrolled shall be maintained by the provider and contain all required information. The record for Child #1 did not contain the following:
Emergency contact information including:
a. Name, home address, and telephone number of each parent who has custody;
b. Name, address and telephone number of each custodial parent's place of employment;
c. Name, office address and telephone number of the child's physician;
d. Name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached;
e. Information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation;
f. Name and policy number of the child's medical insurance
g. Names of persons other than the custodial parents who are authorized to pick up the child;
Plan of Correction: Provider will request the information from the parent.
Standard 8VAC20-800-90-A
Before attendance, documentation of immunizations is to be obtained for each child. There was no documentation of immunizations for Child #1, who was in care during the inspection and had been in care for over 6 weeks.
Plan of Correction: Provider will request a copy of the child's immunization records signed by a physician from the parent.
