Inspector Notes

An announced non-mandated inspection was initiated and completed on 3/23/2026 as a part of the licensure period for the purpose of a request to modify the license to change location. The on-site inspection began at 9:20am and ended at 10am. The inspector reviewed compliance in the areas listed above. There were no children present during the inspection. The inspector reviewed 2 caregiver/household member records. This inspection included document review, tour of the home, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, outside, shall be maintained in a clean, safe, and operable condition. The designated outdoor play area was observed in an unsafe condition. The following items were observed in the outdoor play area: a large metal pole, broken bricks, pile of wooden logs, a ladder, and large sticks scattered throughout the yard.
Plan of Correction: The provider removed all items from the outdoor play area.
Standard 8VAC20-800-320-G
Wood burning fireplaces and associated chimneys shall be inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Documentation of the inspection and cleaning shall be maintained by the provider. A wood burning fireplace was observed in the family day home. There was a wooden log inside of the fireplace. The provider did not have documentation of an inspection and cleaning.
Plan of Correction: The provider reported the fireplace is no longer in use. The provider added a permanent barrier to close off the fireplace.
