During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 8, 2025, as a part of the conditional licensure period. There were four children present and two caregivers; the point total was twelve. The inspector reviewed four children?s records and three caregiver records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-750-A
Topical skin products provided by parents, shall be labeled with the child's name. Insect repellant stored in a child's cubby was not labeled with the child's name.
Plan of Correction: Insect repellant will be labeled with child's name.
Standard 8VAC20-800-750-B
Topical skin products must be inaccessible to children. Insect repellant was observed in a child's cubby within reach of children.
Plan of Correction: Insect repellant will be kept in a locked cabinet.
Standard 8VAC20-800-760-A-1
A first aid kit must be available that has all required components. The first aid kit did not have triangular bandages.
Plan of Correction: Triangular bandages will be purchased.
Standard 8VAC20-800-890
Meals and snacks brought from home must be labeled to identify the owner. Food brought from home for one of the children was not labeled with child's name.
Plan of Correction: Parents will be reminded to label their child's food.
