During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated and completed on 10/09/2025 in response to a complaint received by the licensing office on 10/03/2025 relating to allegations of diapering. The inspector reviewed one children?s record and two staff records on-site.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

When a child's clothing or diaper becomes wet or soiled, the child shall be cleaned and changed immediately.

On 10/02/2025 at 4:30 pm the diaper for child A (age: 3 years) was saturated with urine and soiled from a bowel movement. It was reported that child A's diaper was checked around 4:00 p.m. however, there were no documented diaper changes or diaper checks recorded for child A on 10/02/2025 in the parent communication software.
Plan of Correction: Since October 9, 2025, the leadership team sat with the teachers and unpacked the referred standard together, making clarifications to set straight any misconceptions. We have refined our diaper-changing procedures in that classroom also through retraining, making sure data is recorded to fidelity in the app. Additionally, we designed a Potty Training Individualized Plan document where the school and home will collaboratively design a plan for the child. It will outline the child?s current ability, expectations of school and home, and clearly defined goals for the child. The family will also agree on how they can support potty training at home so that there is continuity of practice.
