During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/30/2025. The on-site inspection began at 10:00 a.m. and ended at 12:10 p.m. The inspector reviewed compliance in the areas listed above. There were seven children present and two staff. The inspector reviewed five children?s records and two staff records on site. This inspection included document review (i.e. daily schedule, license, previous inspection, attendance records, emergency drill logs, and injury reports), tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/08/2025. A POC submitted after this date will not appear on the public website.

Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance. There was no physical evaluation in the record of Child #3 who had been attending the center for seven weeks.

The center shall meet the proof of child identity and age requirements as stated in ? 22.1-289.049 of the Code of Virginia. There was no documentation of viewing proof of the children?s identity or age in the records of Child #1, #3, #4, and #5.

Standard 8VAC20-780-530-C
There shall be at least two staff members with current certification in cardiopulmonary resuscitation (CPR) and current certification in first aid on the premises during the center's hours of operation, on fieldtrips, and wherever children are in care. There were two staff (Staff #1 and Staff #2) present during the inspection. Staff #2 was not certified in CPR or First Aid as required.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all of the required information. The record of Child #4 did not include the phone number of the child's physician or the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. The records of Child #1, #3, #4, and #5 did not contain written agreements between the parents and the center stating that that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff #2, who has been actively working at the center for the past seven weeks, did not have any completed reference verifications on file.
