Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 07/30/2025, as a part of the conditional licensure period. The on-site inspection began at 9:45 a.m. and ended at 12:45 p.m. The inspector reviewed compliance in the areas listed above. There were 3 children present and 5 staff. The inspector reviewed 3 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
6The sworn statement for Staff 4 was dated eight days after their first day of employment.
The national criminal background check for Staff 4 was dated five days after their first day of employment.
There was no documentation of orientation for Staff 1, which is overdue by two months. There was no documentation of orientation for Staff 2, which is overdue by two months. There was no documentation orientation for Staff 3, which is overdue by two months. There was no documentation of orientation for Staff 4, which is overdue by two days. There was no documentation of orientation for Staff 5, which is overdue by two months.
There was no staff member on duty who has obtained instruction in performing the daily health observation.
Three containers of sunscreen were located in the cubbies and accessible to children under the age of five.
The records for Staff 1, Staff 2, Staff 3, Staff 4, and Staff 5 were each missing several of the required components.
