Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. On the outdoor playground area are three large, unlocked storage bins accessible to children which pose as an injury hazard.
Plan of Correction: The center responded with the following: The center will obtain locks for each of the three storage bins on the outdoor playground.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. The mulch was measuring about an inch in depth in the outdoor playground and under the slides the children were landing in patches of dirt and soil.
Plan of Correction: The center responded with the following: The center has a work order to order additional mulch for the playground area and will follow up with the order.
