Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. The program director placed a work order on 10/30/2025 for its repair citing, "it was coming off the wall and seems to be buckling." On October 31st a student knocked into the sink and it separated completely from the wall. The sink had not been repaired or replaced at the time of inspection and was accessible to the children.
Plan of Correction: The director called the district manager during the inspection to track the work order and ensure it will be repaired. In the meantime the sink will be roped off from the children.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. In the three year-old room there were 2 disinfectant sprays in an unlocked cabinet.
Plan of Correction: Chemicals were placed in a locked area.

All record requirements must be kept for each staff person.
1) Staff #1, #2, #3 and #4 did not have documentation of having at least 2 references checked to include who checked the reference, prior to employment.
2) Staff #2, #3, #4 , #5 and #6 did not have documentation of qualifications for their job positions.
Plan of Correction: Someone from corporate office is coming to help the center with records. All of the staff files will be audited.
