Inspector Notes

An unannounced, on-site initial inspection was initiated and completed on June 26, 2025. The on-site inspection began at 10:00am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 25 children present and six staff. The inspector reviewed five children?s records and six staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-3
The facility must request a search of the central registry prior to the employee's first day of employment.

Staff #4, who has been employed ten days, did not have documentation of a central registry search request. Staff #1 was not working alone with children at the time of the inspection.
Plan of Correction: The program director has contacted the Assistant Executive Director to obtain documentation of Staff #4's Central Registry search results.
Standard 8VAC20-780-60-A
Centers must have a separate record for each child enrolled that contains all of the required information.

Child #1, Child #2, Child #3, Child #4, and Child #5's record ( all enrolled two days) were missing the first day of attendance and the schools attended by the children.
Plan of Correction: The program director and camp director will add the first day of attendance and schools attended by the children to the registration form. Moving forward, this information will be added to the registration forms at the time of registration.
