Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 9/23/2025 and completed on 9/23/2025, as a part of the conditional licensure period. The on-site inspection began at 9:30AM and ended at 11:46AM. The inspector reviewed compliance in the areas listed above. There were 16 children present and 3 staff. The inspector reviewed 3 children?s records and 2 staff records on-site on 9/23/2025.
This inspection included document review, tour of the facility and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. A bottle of hand sanitizer with a warning label, ?Keep out of reach of children,? was within reach of children in the 2 year old class.
Plan of Correction: Provider moved all hazardous substances out of the reach of children.
Standard 8VAC20-780-290-A-3
In areas used by preschool age or younger children, electrical outlets shall have protective covers. An outlet in the 2 year old class was not covered.
Plan of Correction: Provider covered all electrical outlets.
Standard 8VAC20-780-500-B
A nonabsorbent surface for diapering or changing shall be used. The diapering surface in the hallway bathroom was torn in 3 places approximately 1 inch in length each.
Plan of Correction: Provider replaced nonabsorbent surface.
Standard 8VAC20-780-550-D
The center shall implement a monthly practice evacuation drill. No evacuation drills were conducted during the month of August 2025.
Plan of Correction: Provider began conducting evacuation drills.
Standard 8VAC20-780-70
The following staff records shall be kept for each person. The record for Staff #1, employed for over a month, did not contain documentation 2 references checked prior to employment. The record for Staff #2, employed for over a month, did not contain job title; date of employment; documentation of 2 references checked prior to employment; documentation to demonstrate education required by the job position.
Plan of Correction: Provider will update staff records.
