During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 7/23/2025. The on-site inspection began at 10:20am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were twelve children present and two staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #5, employed for one month and no longer employed at the center, did not have documentation of requesting a central registry check from one state.
Plan of Correction: The center will ensure that out-of-state background checks are completed.
Standard 8VAC20-780-160-A-2
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #1 and #4 had no documentation and had been employed for one month, and Staff #2 TB was dated eleven months prior to date of employment and has been employed for one month.
Plan of Correction: The center will obtain TB screenings for the staff.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. There was rust found on the classroom air conditioner.
Plan of Correction: The center will have the rust taken care of.
Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #1, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: The center will obtain a medication plan for the child.
