Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/20/2025 as a part of the conditional licensure period. The on-site inspection began at 11:20 am and ended at 1:30 pm. The inspector reviewed compliance in the areas listed above. There were 19 children present and six staff. The inspector reviewed ten childrens records and six staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

The sworn statement for Staff #6 was not fully completed.
Plan of Correction: A fully completed sworn disclosure will be documented in the staff's file before their next shift.

Crib sheets shall be cleaned and washed daily.

Staff reported that crib sheets were being washed weekly.
