The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean condition. In a preschool bathroom there was dried urine on the seat and the rim of a toilet.
Plan of Correction: The facility responded: This bathroom was deep cleaned the evening of the inspection. The new Center Director has since implemented a cleaning schedule for teachers to ensure bathrooms are clean.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Soap labeled "keep out of reach of children" was in a pocket of a rolling bag on the floor of an unlocked bathroom. The bathroom was accessible to the infant and toddler classrooms.
Plan of Correction: The facility responded: This was corrected on the spot during the inspection. We will continue to ensure all hazardous materials are locked away out of reach of children.

The center shall implement a monthly practice evacuation drill. A evacuation drill was not practiced in November 2025.
Plan of Correction: The facility responded: The previous Center Director who is no longer with the company did not complete a drill in the month of November. Drills were done for December and January and moving forward, the plan of action is to continue completing all required drills in the first week of every month.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, enrolled for five months, was missing the work phone number for a parent.
Plan of Correction: The facility responded: This was corrected on the spot during the inspection. The new Center Director is conducting a second file review to verify all required information is completed or marked N/A.
