The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1 did not have documentation of the required immunizations in the record before the child began attending. Immunization documentation was received three weeks after attendance began.
Plan of Correction: The facility responded: Center Director will ensure all children have their Immunization Records on file prior to their first day of attendance.

Physical examination records are required to be signed by a physician, a designee, or an official of the local health department. The physical exam record for Child #1 (enrolled for 12 weeks) was not signed.
Plan of Correction: The facility responded: Center Director will ensure all Physical Exams are properly signed or stamped.

Documentation of negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #1 had no documentation of a TB screening and had been employed for four months.
Plan of Correction: The facility responded: Staff 1 is no longer with the company so we were unable to correct her specific file. Moving forward, Center Director will ensure all staff have their TB screening documented in their file prior to their first day of employment.

Standard 8VAC20-780-500-B
When a child's diaper becomes wet or soiled, the child shall be cleaned and changed immediately. On 11/14/25 Child #1 was in a heavy, soiled diaper when picked up at approximately 2:59pm. The child was wearing the same diaper he had arrived in at approximately 10:00am. Staff #2 stated the diaper the child had on was not one of the company diapers and had not been changed.
Plan of Correction: The facility responded: All staff are trained on diaper changing expectations when they are first onboarded. Children will be checked and changed every 2 hours. Children will also be changed outside of the 2 hour period if they become soiled.
