During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 8/26/25. The on-site inspection began at 10:55am and ended at 3:05pm. The inspector reviewed compliance in the areas listed above. There were 62 children present and 14 staff. The inspector reviewed 7 children's records and 6 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 9/4/25. A POC submitted after this date will not appear on the public website.
Standard 22.1-289.035-B-2
The center must obtain a completed national criminal background check prior to the employee's first day of employment. The national criminal background check results for staff 1 were obtained two weeks after their first day of employment.
Plan of Correction: The center responded with the following: Effective immediately, no employee will be permitted to begin work until a cleared national criminal background check has been received. Hiring staff have been retrained, and the Director will use a pre-employment checklist to ensure compliance moving forward.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child 1, who has been attending for two months, did not have documentation of an immunization record on file.
Plan of Correction: The center responded with the following: The Director will review all current files to confirm immunization records are on file and will ensure that no child is permitted to attend without required documentation moving forward, using an enrollment checklist for verification.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child 1, who has been enrolled for two months.
Plan of Correction: The center responded with the following: Effective immediately, the leadership will ensure that all children?s physicals are received prior to attendance and will be using a
checklist to ensure all documents are received prior to start date.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for child 1, enrolled for two months, was missing documentation of viewing proof of the child's identity and age.
Plan of Correction: The center responded with the following: Effective immediately, leadership will view all children's proof of identity and age before a child's first day of enrollment and document on the correct form.
