An announced, on-site initial inspection was initiated on 5-21-2025 and completed on 5-22-2025. The on-site inspection began at 9:30am and ended at 11:10am. The inspector reviewed compliance in the areas listed above. The inspector reviewed 1 providers record and 3 household member records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5-30-2025. A POC submitted after this date will not appear on the public website.

The Family Day Home center is required to obtain a copy of the results of a sex offender registry check and a search of the child abuse and neglect registry (CPS) or equivalent registry from any state in which the individual has resided in the preceding five years. Household member #2 has lived out of the state and did not obtain an out of state CPS check or sex offender check.

A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation. An operable landline telephone is one that does not require electricity to operate. There was not an operable landline phone available.

Telephone numbers shall be posted in a visible area close to the telephone to include the name and phone number of a person responsible for emergency backup care. There was not an emergency backup person`s name or phone number posted.

The family day home shall have a written emergency preparedness and response plan that includes provisions for a responsible person who is 18 years of age or older and is able to arrive at the family day home within 10 minutes for emergency backup care until the children can be picked up by their parents. The emergency plan did not include an emergency backup person.
