Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8-19-2025 and completed on 8-19-2025, as a part of the conditional licensure period. The on-site inspection began at 2:30pm and ended at 3:40pm. The inspector reviewed compliance in the areas listed above.
There was 1 child present and 1caregiver; the point total was 4. The inspector reviewed 1 child's record,1 staff record and 3 household member records on site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8-28-2025. A POC submitted after this date will not appear on the public website.

Children's immunization records are to contain the dates immunizations were received and shall be signed by a licensed physician, the designee, or an official of a local health department.all required information. The immunization record for Child #1 was missing the physician's signature. The child was present during the inspection and had been in care for a week.
Plan of Correction: Parents were asked to obtain new shot records with doctors signature after upcoming appointment.

Children's records are to contain all required information. The insurance policy number for the child's medical insurance, if applicable and the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. The record for Child #1 was missing the insurance policy number and all the information for the second emergency contact person. The child was present during the inspection and had been in care for a week.
Plan of Correction: Records were immediately fixed. All information was filled in.
