Inspector Notes
An unannounced, on-site initial inspection was initiated on 6/30/25 and completed on 8/07/2025. The on-site inspection on 6/30/25 began at 11:00 a.m. and ended at 1:50 p.m. The on-site inspection on 8/07/25 began at 10:00 a.m. and ended at 11:00 a.m. The inspector reviewed compliance in the areas listed above. Five staff records were reviewed on site. This inspection included document review (i.e. daily schedule, menu, floor plans, policies and procedures, health, food, and fire inspections), tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
