(Repeat Violation) Before the child's first day of attending, parents shall be provided all required information in writing. There was no documentation in the records of Child #1, Child #2, Child #4, and Child #5 that the parents had been provided with required information in writing prior to their children?s first day of attending.
Plan of Correction: Before enrollment all documentation will be given and placed in the children's files
