Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on May 12, 2025. The inspection started at 10:05am and concluded at 11:30am. There were no children present. The inspector reviewed one caregiver records on-site, and five caregiver/household members records electronically on May 6, 2025. Information gathered during the inspection determined non-compliance with applicable standards or law., and violations were documented on the violation notice issued to the program.

Tuberculosis (TB) screening results are to kept for each caregiver. The Provider stated she had completed the screening but did not have documentation at the time of the inspection.
Plan of Correction: My daughter is taking me to get a copy of the results on 5/13/25 and I will send to you.
