During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on September 25, 2025 and concluded remotely on October 6, 2025. The on-site inspection began at 11:39 a.m. and ended at 2:55 p.m. The inspector reviewed compliance in the areas listed above. There were 36 children present and 13 staff. The inspector reviewed 5 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 14, 2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #4 updated screening was due over three weeks ago.
Plan of Correction: Employee received updated TB screening asap and results are in their file. Spreadsheet has been crated to track when file items are due for renewal.
Standard 8VAC20-780-245-J-3
Any child for whom emergency medications have been prescribed shall always be in the care of a staff member or independent contractor who meets the requirements to administer the medication. There are no staff certified to administer an emergency medicine prescribed to child #6, who had been enrolled for over a month.
Plan of Correction: Multiple staff members have now received the mandated training for the specific medication. Center will continue to ensure that enough staff have required training for all emergency medications in the center.
Standard 8VAC20-780-280-B
Repeat Violation
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
1. There were two bottles of bleach water in an unlocked cabinet under the sink in the infant room. The cap was not tightly on one bottle and bleach water spilled onto the inspector?s hand. These bottles were within reach of children.
2. There were Lysol wipes on a low shelf in a hallway bathroom.
Plan of Correction: All employees were reminded to keep hazardous substances out of reach of children and locked away after each use. Further training for all staff will occur at the staff meeting on 10/14/2025. Lysol wipes were removed from adult bathroom immediately.
Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff. During the inspection, three children (age 2 years) were out of actual sight from Staff #1 for several minutes. Staff #1 was at the diapering table, which is out of sight, from the main portion of the classroom. Staff #1 was changing diapers for several minutes and could not see three children that were around the corner in the main area of the classroom.
Plan of Correction: A work order has already been placed to install supervision mirrors in the classroom to allow better sight supervision. Staff have also been reminded to move changing table when necessary.
Standard 8VAC20-780-500-A
Children?s hands shall be washed with soap and running water after toileting and staff?s hands shall be washed with soap and running water before and after a diaper change.
1. Child #4 did not wash hands after toileting.
2. Staff #1 did not wash hands before changing two children.
Plan of Correction: All teachers have been provided with the handwashing procedure. A re-training is also occurring during the staff meeting on 10/14/2025. Observations will be conducted weekly to ensure follow through.
Standard 8VAC20-780-500-B
The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. Staff #1 changed a diaper and did not clean and sanitize the diapering surface after use and immediately changed another child?s diaper. Staff #1 said sanitizing isn?t done until all diapers are changed.
Plan of Correction: All teachers were provided with the diaper changing procedures. A re-training is also occurring during staff meeting on 10/14/2025. Observations will be conducted weekly to ensure follow through
Standard 8VAC20-780-70
The center shall keep staff records with all the required information for each staff person. 1. The center did not have staff records for staff #5, who had been employed for approximately a month, and staff #6, who had been employed for over a month.
2. References for staff #5 were checked four days after employment and references for staff #6 were checked one month after employment.
3. Staff #1, identified as a program leader, did not have program leader qualifications on file.
Plan of Correction: All substitute staff will have physical copies of files at center their first day of work on site. Staff will ensure references are completed and filed prior to the first day of employment. Qualified program teachers will have documentation in files to support job title.
