The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use and disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
1. Staff #1 did not sanitize the diaper changing surface after changing a diaper. The inspector had to intervene before another child was changed.
2. The foot-operated diaper storage system in the infant room was broken and staff used their hands, touching the exterior surface of the storage system.

Plan of Correction: Medication authorization forms for both children have been received at the center. Center leadership will ensure that any medication forms are received before a child needs medication kept at the center.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and contain all the required information. Incident reports reviewed did not contain the date and time the parent was notified and documentation of how the parent was notified.
Plan of Correction: Teachers will be retrained on ensuring all parent notification information is added to incident reports - including date, time, and method of notification.

When food is brought from home unused portions of opened food shall be discarded by the end of the day or returned to the parent. Baby food in a Tupperware container was still in the infant 1 refrigerator two days after it was dated.
Plan of Correction: All bottles and food will be either discarded or returned to families at the end of each day at pickup.

Plan of Correction: All infant families will be re-notified of bottle labeling procedures and expectations. All infant teachers will be re-trained on bottle labeling as well as checking all bottles when they enter each morning and returning them to families if not correct.
