REPEAT VIOLATION
SYSTEMIC VIOLATION
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
1. In the infant room, staff #2, left bleach water in an unlocked cabinet accessible to children, while cleaning the table.
Plan of Correction: Continue training with staff member on completely shutting locked cabinet doors, even when standing nearby. Also working with facilities on shelf relocation in cabinets so one at child level doesn't need used [sic].

SYSTEMIC VIOLATION
Children's hands shall be washed with soap and running water or disposable wipes before and after eating and staff shall wash hands with soap and running water before and after diapering and before feeding or helping children with feeding, and preparing or serving food or beverages.
1. Infant #1 and infant #2's hands were not washed before and after eating.
2. Staff #1 did not wash hands before diapering and before feeding.
3. Staff #2 did not wash hands before preparing food and before feeding.
Plan of Correction: Continue training with all infant teachers on hand washing procedures and WHEN hand washing must occur for all adults and children. Installing new signage that will aid in the training and reminders to staff of all the handwashing points during the day. Continue to reinforce handwashing with all staff at upcoming staff meeting in February and March.

Tables and high chair trays shall be sanitized before and after each use for feeding.
1. The table and high chair trays were not sanitized in the infant room after each use.
Plan of Correction: Continue training with all infant teachers on the sanitizing of tables and trays both before and after each use.
