Inspector Notes

An announced, on-site initial inspection was initiated and completed on June 11, 2025. The on-site inspection began at 12:07 p.m. and ended at 1:20 p.m. The inspector reviewed compliance in the areas listed above. The inspector reviewed 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 26, 2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-2
The center must obtain a completed national criminal background check prior to the employee's first day of employment. An individual listed on the initial application did not have a completed national criminal background check.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. There was chipped and peeling paint in nearly every classroom of the center and on the two's playground fence and tricycles. Several classrooms and bathrooms had wall damage that exposed drywall or created a hole in the wall, there was missing caulking in several areas, and there was a large amount of visible clumped dirt/dust in an open windowsill accessible to children.
Plan of Correction: The center is currently operated by another provider. Once access is available, we will repair all facilities and maintenance items listed.
Standard 8VAC20-780-40-N
The center shall develop written playground safety procedures which shall include the method of maintaining resilient surfacing. The center's procedures do not include the method to maintain resilient surfacing.
Plan of Correction: The center has a playground daily inspection checklist which includes a daily inspection of fall zones, holes, and tripping hazards. Any deficiency is immediately reported to facilities for repair.
Standard 8VAC20-780-550-A
The center shall have a written emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation and relocation, shelter-in-place, and lockdown. The plan, which shall be developed in consultation with local or state authorities, shall include the most likely to occur emergency scenario or scenarios. The center did not have a completed emergency preparedness plan developed in consultation with local or state authorities.
Plan of Correction: An updated emergency response plan is being updated with the new center operations and leadership team and will be in place before the center opens. The plan will be reviewed by local authorities. Staff are receiving training on 6/28/25 prior to the opening date.
Standard 8VAC20-780-550-B
The emergency preparedness plan shall contain procedural components for emergency communication, evacuation, shelter-in-place, lockdown, and continuity of operations. The center's emergency plan did not contain the name and phone number of a secondary emergency officer, the assembly points, the primary and secondary means of egress, securing information on allergies or food intolerances, and the continuity of operations to ensure that essential functions are maintained during an emergency.
Plan of Correction: An updated emergency response plan is being updated with the new center operations and leadership team and will be in place before the center opens. Staff are receiving training on 6/28/25 prior to the opening date.
