63.2 Child Abuse & Neglect
Inspector Notes
An unannounced monitoring inspection was initiated remotely on July 7, 2025 and completed on August 27, 2025, with child protective services, in response to a self-report received by the licensing office on July 3, 2025 relating to supervision of children. The inspector reviewed two children's records and two staff records electronically. This inspection included document review and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to background checks and mandated reporting and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 4, 2025. A POC submitted after this date will not appear on the public website.
Standard 22.1-289.036-B-2
The center must obtain a completed national criminal background check prior to the employee's first day of employment. Fingerprint results were obtained 18 days after staff #2 was employed and four days after staff #1 was employed.
Plan of Correction: All background checks were up-to-date with prior vendor. New ones were needed simply due to the change in operator and while submitted, had not all been received back timely. All new hires will have these completed prior to their hire date and do not start work without them being reviewed.
Standard 63.2-1509-A
Staff employed by a child day program who have reason to suspect that a child is abused or neglected shall report the matter immediately to the local department's toll-free child abuse and neglect hotline. Staff became aware of a reportable offense on July 3, 2025, and failed to report to the local department.
Plan of Correction: Center leadership immediately notifed parents and self-reported to licensing. A report was not made to the child abuse and neglect hotline as the children were safe, unharmed, and not upset. We did not suspect abuse or neglect. Moving forward, the center will report all abuse/neglect suspicions as required.
Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff. On July 3, 2025, child #1 (4yrs old) and child #2 (4yrs old) were left alone outside on the playground for approximately five minutes.
Plan of Correction: All staff are being retrained on transition tracking policies-including conducting a final visual sweep of spaces. Leadership team is conducting checks to ensure compliance.

The operational responsibilities of the licensee shall include ensuring that the center?s activities, services, and facilities are maintained in compliance with these standards, the center?s own policies and procedures that are required by these standards, and the terms of the current license issued by the department. Staff did not follow the center?s procedures for identifying where attending children are at all times when staff #1 and staff #2 failed to complete the center?s name-to-face child count when leaving a location and entering a new location, leaving two children (age 4), outside on the playground alone for approximately five minutes.
Plan of Correction: Both staff members have been retrained on tracking procedures and expectations. CD and LE are conducting checks to ensure compliance with policies as well. A center leader is observing all transitions to/from classrooms and playground until training and all observations are complete.
