Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. The kitchen door was not locked propped open and there was a gallon container of germicidal bleach, three gallons of sanitizer, and three gallons of liquid detergent on the floor and accessible to children.
Plan of Correction: Center leadership purchased a lock for the kitchen to prevent access by children and hazardous substances were moved to a locked cabinet within the kitchen.

Children under 10 years of age shall always be within actual sight and sound supervision of staff. During the inspection, child #3 (age 4 years) was out of actual sight from both staff for three minutes, until the licensing inspector intervened.

Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Used diapers were located in an open trashcan that wasn?t leakproof nor a foot-operated system.
Plan of Correction: Staff will be retrained on proper diapering procedure and policies. Center leadership will also conduct diapering checks when entering classroom for ratio checks.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and contain all the required information. Three incident reports reviewed did not contain the date and time the parent was notified, documentation of how the parent was notified, and two staff and parent signatures.
Plan of Correction: Center leadership team (CD, AD, LE) will review all incident reports before printing to ensure required information is documented.

A written agreement between the parent and the center shall be in each child?s record by the first day of the child?s attendance. Child #2 was enrolled for over a month before the center obtained a written agreement from the parent.
