Inspector Notes
An announced, on-site initial inspection was initiated on 8/14/2025 and completed on 8/15/2025. The on-site inspection began at 10:00 am and ended at 11:20 am. This inspection included document review, tour of the home, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/27/2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-800-280
Poisonous materials shall be stored in an area inaccessible to children. Poisonous cleaning chemicals were in a cabinet below the sink in the kitchen, accessible to children.
Plan of Correction: The cabinet was locked, during the inspection, making them inaccessible to the children.
Standard 8VAC20-800-330-A
A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation. An operable landline telephone is one that does not require electricity to operate. Cordless or cell phones may be used in addition to the landline telephone. There was no landline telephone available.
Plan of Correction: Provider will have landline by 8/29/2025.
Standard 8VAC20-800-350-E
Hot water at taps available to children shall be maintained within a range of 105?F to 120?F. The hot water temperature at the sink in the bathroom registered at 122 F.

The provider shall maintain documentation of the current rabies vaccination. One of the family pets rabies vaccination was past due by two months.
Plan of Correction: Vaccination was given on 8/17/2025.

A first aid kit shall be in the family day home and contain all required items. There were no triangular bandages.
