During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 10/14/2025. The on-site inspection began at 12:00 pm and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver; the point total was 16. The inspector reviewed 3 children?s records and 2 caregiver/household member(s) records on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/22/2025. A POC submitted after this date will not appear on the public website.
Violations
21. Name, address and telephone number of each custodial parent's place of employment;
2. The address for the 2nd emergency contact;
3. Names of persons other than the custodial parents who are authorized to pick up the child;
4. First date of attendance;
5. Proof of the child's age and identity and the names and addresses of previously attended child day care and schools;
6. Written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds;
1. Policies for the administration of medications;
2. Whether or not there is liability insurance;
3. Requirement for the family day home to notify the parent when the child becomes ill;
4. Requirement for paid caregivers to report suspected child abuse or neglect;
5. Custodial parent's right to be admitted to the family day home any time the child is in care;
6. Presence of a pet or animal in the home;
7. Amount of time per week that an adult assistant or substitute provider instead of the provider is scheduled to care for the child and the name of the adult assistant or substitute provider;
8. Provisions of the family day home's emergency preparedness and response plan;
9. Parental notifications required in 8VAC20-800-650;
10. Address of the website of the department, with a note that a copy of this chapter and additional information about the family day home may be obtained from the website, including compliance history.
