The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There were no physical exam records available for Child #3, #4, & #5.

The record for Child #4 was missing the name, address, and telephone number for one parent's place of employment; name and phone number for the child's physician; name, address, and phone number for two emergency contacts; names of persons other than the custodial parents who are authorized to pick up the child; and the first date of attendance. The record for Child #5 was missing the name, address, and phone number for one emergency contact; and the first date of attendance.

Before the child's first day of attendance, parents shall be provided in writing all required information and shall obtain the parent's written acknowledgement of the receipt of the information. Child #3 was missing all 19 required information items. Child #4 was missing the family day home's emergency preparedness and response plan. Child #5 was missing the family day home's emergency preparedness and response plan and policies for the administration of medications.
Plan of Correction: Parents will resign missing documents that were provided by provider.

The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care. There were no drills practiced for November 2025, December 2025, or January 2026.
