The center is required to obtain a completed sworn statement prior to the employee's first day of employment. There was no sworn statement for Caregiver #2.

The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There were no physical exam records available for Child #3, #4, & #5.

Assistants' and substitute providers' records shall contain all required information. The record for caregiver #2 was missing the name, address, and telephone number of an emergency contact; documentation of two or more references as to character and reputation; and training regarding the emergency preparedness plan and procedures.

Providers must obtain a current, negative, tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children. There was no tuberculosis screening for Caregiver 2.

A child's record shall contain all required information. The record for Child #3 was missing the address for both parents; address and telephone number of each parent's place of employment; name and phone number for the child's physician; first date of attendance; and proof of the child's age and identity. The record for Child #4 was missing the name, address, and telephone number for one parent's place of employment; name and phone number for the child's physician; name, address, and phone number for two emergency contacts; names of persons other than the custodial parents who are authorized to pick up the child; and the first date of attendance. The record for Child #5 was missing the name, address, and phone number for one emergency contact; and the first date of attendance.

Before the child's first day of attendance, parents shall be provided in writing all required information and shall obtain the parent's written acknowledgement of the receipt of the information. Child #3 was missing all 19 required information items. Child #4 was missing the family day home's emergency preparedness and response plan. Child #5 was missing the family day home's emergency preparedness and response plan and policies for the administration of medications.

The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care. There were no drills practiced for November 2025, December 2025, or January 2026.

Temperatures shall be maintained at or below 40?F in refrigerator compartments and at or below 0?F in the freezer compartments. The temperature in the freezer was 11 degrees.
