Inspector Notes

An announced, on-site initial inspection was completed on 06/25/2025, as a part of the initial licensure period. The on-site inspection began at 10:00 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 2 staff. The inspector reviewed 3 children?s records and 2 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition.

There was a missing board on the climbing portion of the play equipment outside.
Plan of Correction: Provider will repair the climbing wall.
Standard 8VAC20-800-70-A
Before the child's first day of attendance, parents shall be provided in writing the provisions of the family day home's emergency preparedness and response plan.

The children's records did not contain the provisions of the family day home's emergency preparedness and response plan.
Plan of Correction: Provider will ensure parents receive provisions of the family day home's emergency preparedness and response plan in writing.
Standard 8VAC20-800-730-D
Medication, except for those prescriptions designated otherwise by a written physician's order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children.

Household medications were in a closet in the restroom that the children in care utilize. This closet had a twist lock but it would not prevent children from entering the closet.
Plan of Correction: Provider will ensue that medications are locked in a manner that prevents access by children.
