Standard 22.1-289.035-B-1
Each staff shall complete a sworn disclosure statement by the first day of employment. Staff 1 (employed for two months), Staff 3 (employed for 2 months), and Staff 5 (employed for 4 months) signed the sworn disclosure statement, but did not document the date completed.
Plan of Correction: Background checks will be signed and dated as required.

There was no documentation that the central registry search for Staff 1 and Staff 5 were requested prior to employment as required. The searches had not been returned as of November 24, 2025. Staff 1 was left alone with children during today's inspection.

Standard 8VAC20-780-240-A
Staff shall complete the Virginia Department of Education-sponsored orientation course (VA Pre-service) within 90 days of employment. Staff 5 (employed 4 months) did not have documentation of having completed the VA Dept of Education sponsored orientation course within 90 days of employment.
Plan of Correction: Staff will complete the VA Preservice training as soon as possible.

Documentation of orientation training shall be kept by the center in a manner that allows for identification by individual staff member, is considered part of the staff member's record, and shall include: the date of training. Orientation for Staff 1 (employed for 2 months) and Staff 2 (employed for over one month), Staff 3 (employed for 2 months), and Staff 5 (employed for 4 months) were not dated.

There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. The three staff on site did not have documentation of daily health observation training.

The most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA) for snack recommends two components from two different food groups. The facility only served one component from one food group for afternoon snack today. Animal crackers and water were served.
Plan of Correction: Two components will be served for snack daily.

The facility failed to have required documentation in each staff file. Two references as to character, reputation, and competence should be checked prior to employment. Staff 1, Staff 2, and Staff 3 did not have documented dates of when phone references were completed.
