Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/15/2025 and completed on 9/15/2025, as a part of the conditional licensure period. The on-site inspection began at 9:10AM and ended at 10:55AM. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 staff. The inspector reviewed 4 children?s records and 1 staff record on-site on 9/15/2025.
This inspection included document review, tour of the facility and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/25/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards. The resilient surfacing around the fall zones of the playground structure measured 6 inches.
Plan of Correction: Provider will ensure resilient surfacing is correct depth.
Standard 8VAC20-780-560-G
When food is brought from home the food container must be clearly labeled and dated. 11 children?s lunches were not dated in classroom 1 and 7 children?s water bottles were not labeled with names in classroom 1.
Plan of Correction: Provider will date food brought from home.

Tables and high chair trays shall be sanitized before and after each use for feeding. 2 children were observed eating snack and when they finished the table was not sanitized before 2 more children began eating snack.
Plan of Correction: Provider will sanitize tables after children eat.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #4 was missing the following required component: first date of attendance.
