Documentation of orientation training shall be kept by the center in a manner that allows for identification by individual staff member. The record for staff #5 did not have documentation of completion of orientation.

The center shall maintain, in a way that is accessible to all staff who work with
children, a current written list of all children's allergies, sensitivities, and dietary
restrictions documented. This list shall be dated and kept confidential in each room or area where children are present.

There was not allergy, dietary restriction and sensitivity list in the center. Child #2, child #4 and child #5 had an allergy,dietary restriction or sensitivities listed in their record.

The required first aid kits shall include an antiseptic cleansing solution /pads.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Attendance was documented as one group. The children were separated in groups. The attendance was not separated in groups.

All staff who work directly with children shall have within 90 days of the date of
employment or 90 days from subsidy vendor approval a current certification in cardiopulmonary resuscitation(CPR) appropriate to the ages of children in care. The training shall include an in-person competency demonstration.

Staff #6 CPR documentation provided expired a year ago. Staff #1 does not have CPR certification with training that includes a inperson competency demonstration. Staff #1 should have completed with in 90days from subsidy approval from 6 months ago.
