During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An announced, on-site initial inspection was initiated and completed on 6/17/25. The on-site inspection began at 12:10pm and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were zero children present and three staff. The inspector reviewed zero children?s records and zero staff records. This inspection included a tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/2/25. A POC submitted after this date will not appear on the public website.

Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition.
1.There was a chair on the outdoor play area that was frayed and ripped, making it inoperable.
2. On the outdoor play area, there was a tree stump protruding approximately two inches, posing a tripping hazard.
3. There was a bucket of standing water on the outdoor play area. Mosquitoes were observed in the water.
4. In two bathrooms that children will use, there were screws protruding on the sides of two toilets, posing an injury hazard.
Plan of Correction: 1. The chair was removed at the time
of the inspection.

4. The screws have been cut down.
