The following actions or threats thereof are forbidden: roughly handling a child.
On October 9, 2025, Staff #1 grabbed Child #1 by the shoulders and pushed them causing Child #1 to fall backwards and to hit their head on the carpeted floor.
Plan of Correction: 1. Immediate Removal of Involved Staff
On October 9th, as soon as another staff member reported the incident, the staff member involved was immediately removed from the classroom and placed in the office. Moving forward, this same immediate removal procedure will be followed for any similar report to ensure the safety of all children.
2. Immediate Notification to Human Resources
On October 9th, Administration contacted HR immediately upon being informed of the incident. This practice will continue for all future incidents to ensure proper guidance, documentation, and adherence to personnel procedures.
3. Internal Interviews and Documentation
Following the October 9th report, Administration conducted a full internal review by interviewing the staff member involved and the staff member who witnessed the incident. Detailed notes were taken to capture the facts of the situation. This process will remain in place for any future incidents to ensure accurate, timely documentation.
4. Administrative Action Based on Findings
After gathering information on October 9th, HR and Administration jointly made the decision to terminate the staff member involved. This action was carried out immediately, and the staff member was escorted to gather her belongings and leave the premises. In all future cases, appropriate disciplinary action-including immediate termination when warranted-will be taken based on the findings of the internal review.
5. Required Reporting to DSS and Licensing
On October 9th, after completing the internal assessment, Administration made mandatory reports to the Department of Social Services and Licensing. These agencies took over the investigation from that point forward. Administration will continue to promptly notify all required agencies of any similar incidents going forward.
6. Parent Notification
Administration notified the parents of the child involved on the same day, informed them of the situation, and let them know that DSS and Licensing had been contacted and would continue the investigation. This procedure will continue to be followed for all future incidents involving a child.
7. Ongoing Staff Training and Reinforcement
Based on the events of October 9th, Administration will reinforce training for all staff regarding appropriate staff-child interactions, professionalism, supervision expectations, and mandatory reporting procedures to prevent similar incidents from occurring.
8. Increased Monitoring of Classrooms
As a corrective response to the October 9th incident, Administration will increase monitoring and walkthroughs in classrooms to ensure all staff are following expectations and that children remain safe at all times.
