Inspector Notes
An announced, on-site monitoring inspection was initiated on 7/18/2025 and completed on 7/18/2025, as a part of the conditional licensure period. The on-site inspection began at 9:55AM and ended at 12:00PM. The inspector reviewed compliance in the areas listed above. This inspection included document review, tour of the facility and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a safe condition. A hole in the wall above the urinal in the Room A and B bathroom had exposed insulation. The Room C and D bathroom had broken metal brackets on the wall with sharp edges.
Plan of Correction: Provider will cover hole in wall and remove broken metal brackets.
Standard 8VAC20-780-290-A-3
In areas used by preschool age or younger children, electrical outlets shall have protective covers. An outlet in Room 1, Room 4 and Room A did not have a protective cover.
Plan of Correction: Provider will place covers on outlets.
Standard 8VAC20-780-540-C
First aid kits shall include all required items. The center?s first aid kit was missing the following required items: a manual and thermometer.
Plan of Correction: Provider will purchase all required first aid supplies.
Standard 8VAC20-780-540-E
The following nonmedical emergency supplies shall be required. The center?s emergency supplies did not include a battery-operated radio.
Plan of Correction: Provider will purchase a battery operated radio.
