Inspector Notes
An announced, on-site initial inspection was initiated and completed on July 9, 2025. The on-site inspection began at 9:40am and ended at 10:55am. The inspector reviewed compliance in the areas listed above. The inspector reviewed one staff record on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening completed within the last 30 calendars days of the date of employment. The screening for Staff #1 was dated over a year prior.
Plan of Correction: A new TB screening will be completed today.
Standard 8VAC20-780-500-B
Diapering areas shall have a sink with running running water, a non-absorbent surface for changing a diaper and a diaper disposal that does not require a hand or diaper to touch the exterior to operate. In the yellow room, there was no sink in the diapering area, the diapering surface was covered with a cloth cover causing the surface to be absorbent and a swivel top diapering pail was for diaper disposal.
Plan of Correction: The diapering table was moved to the bathroom beside a sink. The cloth cover on the diapering surface was removed during inspection. A foot operated disposal system was moved to the diapering area during inspection.
